3. Fill out the form below to report the change AFTER opponent,
site, and assignor all agree on new date/time. |

4. Game changes made after 8-21-2008 will incur additional
referee fees being paid by the requesting team. When games are
moved, the team requesting the change will pay the referees for the
new date and/or time. |
